CPT Code Description: Emergency department visit for the evaluation and management of a patient with a problem of moderate severity – presenting within 30 minutes
Report:
The patient, John Smith, a 53-year-old male, presented to the emergency department with complaints of chest pain that began approximately one hour prior to arrival. The patient reported the pain was located in the center of his chest and radiated to his left arm. The patient rated the pain 8 out of 10 in intensity and described it as a crushing sensation.
Upon arrival, the patient was immediately triaged and taken to a room for evaluation. A thorough history was obtained, including any pertinent past medical history, family history, and social history. A focused physical exam was performed, which included an assessment of the patient’s vital signs, a cardiovascular exam, and a respiratory exam. The patient’s vital signs were stable, with a blood pressure of 132/78, a heart rate of 88 beats per minute, a respiratory rate of 18 breaths per minute, and an oxygen saturation of 98% on room air.
Based on the patient’s history and physical exam, the working diagnosis was acute coronary syndrome, and the patient was treated accordingly. A 12-lead electrocardiogram was obtained, which showed ST segment elevation in leads II, III, and aVF, consistent with an inferior myocardial infarction. The patient was given aspirin, nitroglycerin, and heparin, and arrangements were made for emergent cardiac catheterization.
Overall, this was a moderate severity encounter, requiring immediate attention and intervention. CPT code 99283 is appropriate for this encounter, given the urgency of the situation, the thorough evaluation and management provided, and the need for timely intervention.
